Which primary care activities are associated with reduced emergency hospital admissions in the last months of life, and for whom are these activities likely to be most effective?

This project will use routine data linkages to secondary data analysis to understand “Which primary care activities are associated with reduced emergency hospital admissions in the last months of life, and for whom are these activities likely to be most effective?” for adults and children and young people with life-limiting illnesses.
