This dataset supports the study "Modular organization of the cognitive connectome predicts conversion to Mild Cognitive Impairment." It comprises baseline and three-year follow-up data from 822 community-dwelling, cognitively healthy older adults (aged 65–87 years at baseline), of whom 115 converted to mild cognitive impairment (MCI) during follow-up. Data were drawn from two harmonized Spanish ageing cohorts (Elderly Relatives of Alzheimer's Disease [ERAD] and MADRID+90). The deposit includes: (i) a de-identified participant-level data table with raw neuropsychological test scores (20 cognitive variables spanning global cognition [MMSE], verbal episodic memory [FCSRT], visuoconstruction/visual memory [ROCF], confrontation naming [BNT-15], semantic and phonemic verbal fluency, working memory and processing speed [Digit Span, Digit Symbol], and clock drawing); (ii) demographic and clinical covariates (age, sex, education level, GDS-15, STAI-trait, APOE ε4 genotype group, and subjective cognitive decline [SCD] status); and (iii) the time-to-event variables (follow-up time and MCI conversion status). All data are fully anonymized; no direct or indirect identifiers are included. Participants provided written informed consent, and the study was approved by the Research Ethics Committee of the Hospital Universitario Clínico San Carlos (Madrid, Spain) and the Ethics Committee of the Carlos III Institute of Health (Madrid, Spain). in accordance with the Declaration of Helsinki. These data enable reproduction of all reported analyses and support reuse in research on cognitive ageing, network neuropsychology, and prognostic modelling of MCI conversion.
