Whether each of the 51 US Medicaid programs (the 50 states and the District of Columbia) covers GLP-1 drugs such as Wegovy, Zepbound and Foundayo for weight loss, as of Oct 18, 2026. Each row gives the coverage status (Covered, Covered with limits, Ended in 2026, Not covered, Not confirmed), the drugs and prior authorization rules, coverage of Wegovy for heart risk and of Zepbound for obstructive sleep apnea, the latest 2025 or 2026 policy change with its date, and the URLs of the primary documents behind the row (preferred drug lists, prior authorization criteria, provider bulletins, state rules and legislation). As of the check, 10 programs cover a GLP-1 for weight loss (7 fully, 3 with limits) and 7 ended coverage during 2026. One CSV (11 columns, UTF-8, ISO 8601 dates) and a README data dictionary.
