Each midwifery patient's details are recorded in single page column format, with the entries arranged by date of admission. Each entry included a space for the hospital's Bradma label with the patient U. R. [Universal Record] number, address and name of spouse / partner.The birth information listed under the following column headings comprise: Parity Gest [Gestation] W/ks [weeks]; Labour; Analgesia; Delivery 1 2 3; Time & Sex; Weight & Length; Third Stage; Accoucheur [obstetrician and or midwife]; Remarks. After the columns there is a section where summary information is recorded e.g. number of births on that page.

所有助产产妇的详细信息均采用单页栏位格式记录，条目按入院日期排序。每条记录均预留医院布拉德马（Bradma）标识栏，用于填写产妇通用记录（Universal Record，U.R.）编号、住址及配偶/伴侣姓名。以下列标题对应的分娩相关信息包括：产次、妊娠（Gestation，Gest）时长、孕周（weeks，W/ks）、产程、镇痛方式、第1、2、3次分娩、分娩时间与胎儿性别、新生儿体重与身长、第三产程、接诊医师/助产士（Accoucheur，即产科医师或助产士）及备注。栏位之后设有摘要信息记录区，用于记录诸如当前页面的分娩总次数等汇总内容。
