Background: Polysulfone (PS) membranes are the predominant material in modern hemodialysis (HD), yet rare hypersensitivity reactions remain a clinical concern. This retrospective cohort study aimed to characterize the clinical presentation and immunotoxicological correlates of PS hypersensitivity and adapts novel laboratory tests for its detection. Methods: Eighty HD patients were analyzed between 2018 and 2022, including 51 with clinical suspicion of PS hypersensitivity and 29 asymptomatic controls. Clinical data were collected using a structured symptom survey, and blood samples were analyzed for Type I (basophil degranulation test, BDT) and Type IV (lymphocyte transformation test, LTT; specific lymphogenesis) immune reactivity. Statistical analysis included χ² and Fisher’s exact tests, ROC analysis, and network analysis of symptom clusters.
