OCHIN Community Health Database - AIM-AHEAD Year 4

The AIM‑AHEAD OCHIN Community Health Database is a longitudinal electronic health record dataset derived from OCHIN’s national network of community‑based health centers and standardized to the Observational Medical Outcomes Partnership (OMOP) Common Data Model version 5.4.1 and OHDSI's February 2024 Vocabulary Release. The database represents an open cohort of patients with at least one ambulatory, telehealth, or dental visit and includes records from 2012 through 2024, reflecting the growth and evolution of participating clinics over time. Available data span key clinical and administrative domains, including patient demographics, visits, conditions, procedures, medications, laboratory results, vital signs, immunizations, and patient screenings. The dataset also includes mortality information from EHR documentation and privacy‑preserving linkage to external mortality sources, supporting longitudinal and population‑based research across safety‑net settings.
