**Overview**
This data package contains information about Measures of Rehospitalization, Emergency Visit and Community Discharge for Medicare Beneficiaries. It also includes Nursing Home Compare information on Deficiencies, Fire Safety Deficiencies, MDS Quality Measures, Ownership information, Fines and Payment denial, Provider Information, State Averages and Survey Summary information about nursing homes.

**Description**
This data package contains the Measures of Rehospitalization Emergency Visit and Community Discharge are based on Medicare claims data. These three quality measures measure the rate of rehospitalization, emergency room use, and community discharge among nursing home residents. They include Percentage of short-stay residents who were successfully discharged to the community, Percentage of short-stay residents who have had an outpatient emergency department visit and percentage of short-stay residents who were re-hospitalized after a nursing home admission. Nursing Home Compare Deficiencies currently listed on Nursing Home Compare. Nursing Home Compare allows consumers to compare information about nursing homes. It contains quality of care and staffing information for all 15,000 plus Medicare- and Medicaid-participating nursing homes. Nursing Home Compare Fire Safety Deficiencies, MDS Quality Measures, Ownership, Penalties, Provider Information, State Averages and Survey Summary datasets allows consumers to compare information about nursing homes. Nursing Home Compare includes information on Health and fire-safety inspections with detailed and summary information about deficiencies found during the 3 most recent comprehensive inspections (conducted annually) and the last 3 years of complaint investigations and a set of quality measures that describe the quality of care in nursing homes including percent of residents with pressure sore, percent of residents with urinary incontinence and more and 5-star quality ratings of individual or overall performance on health inspections, quality measures, and hours of care provided per resident by staff performing nursing care tasks and the nursing home staffing information about the number of registered nurses, licensed practical or vocational nurses, physical therapists and nursing assistants in each nursing home.
