This paper illuminates a serious gap in clinical knowledge: the systematic neglect of the toxicity of non-transferrin-bound iron (NTBI) in modern medicine. As a biological axiom, it is indisputable that when transferrin saturation exceeds 50%, NTBI forms, causing oxidative stress and cellular damage. Special attention is given to H63D syndrome and particularly Type-3 (Oshtoran syndrome), a constellation of NTBI accumulation, autonomic dysregulation, and immune system disorders with severe systemic effects. This multisystemic disease manifests in various organ damages and is characterized by its high clinical volatility. The urgent appeal to the medical community is to abandon its intellectual complacency and integrate current research findings into clinical practice to prevent unnecessary suffering.
