This study investigated the impact of the Intravenous to Per Oral Switch (IV-to-PO switch) program in a tertiary care hospital in Islamabad, Pakistan. The primary goals were to assess the effect on length of hospital stay and cost savings, with secondary objectives focused on missed opportunities and usage patterns. Although the IV-to-PO switch did not significantly affect the length of stay, it yielded an 8.7% reduction in total pharmacy costs (PKR). The majority of switches, particularly in the obstetrics/gynaecology specialty, involved transitioning from ceftriaxone to cefixime. This highlighted the program's potential for cost savings and its positive implementation impact.
