NRS-21500 | Patient index cards - Fire files [Morisset Hospital]

These are Patient index cards relating to files that were destroyed in a fire at Morisset Hospital in the mid 1960s. They relate to patients admitted to Morisset Hospital for the Insane, later known as Morisset Mental Hospital and Morisset Hospital, prior to 1966. There are no patient medical records connected to this series.The amount of information recorded on the cards varies depending on period and card type. Early cards, up until the 1920s, routinely record name, register number, date admitted, religion and details of friends, and sometimes bore additional annotations (e.g. age, social condition, discharge, transfer). From the late 1920s, the cards provide additional fields to record: date of birth, social condition (married, single etc.), number of children, occupation, nationality, address, mode of admission (voluntary or certified), date of discharge or death, special information for reports (e.g. details of leave), and date of discharge or death. From 1965, cards were printed by a mimeographic (Roneo) process and, in addition to the above, record: pension status, previous outpatient psychiatric treatment, number of previous admissions, referred by, source of referral, ward, medical officer, type of admission (first or previous), classification of patient (including re-classifications), and disposal (e.g. order expired, hospital transferred to).The cards are arranged alphabetically by name in separate male and female sequences.

卡片所记录的信息详略程度因制作时期与卡片类型有所差异。20世纪20年代以前的早期索引卡通常常规记录患者姓名、登记号、入院日期、宗教信仰及亲友信息，部分卡片还会附带额外标注，例如年龄、社会状况、出院及转院情况。20世纪20年代末起，卡片新增了以下记录字段：出生日期、婚姻状况（已婚、未婚等）、子女数量、职业、国籍、住址、入院方式（自愿入院或强制入院）、出院或死亡日期、用于报告的特殊信息（例如休假详情），以及出院或死亡日期。1965年起，卡片采用油印（mimeographic，罗内奥（Roneo））工艺制作，除上述字段外，还新增记录以下内容：养老金领取状态、既往门诊精神科治疗史、既往入院次数、转介人、转介来源、所在病房、接诊医师、入院类型（首次入院或再次入院）、患者分类（含重新分类）及处置情况，例如医嘱到期、转至其他医院等。本批索引卡按患者姓名的字母顺序排列，分为男性患者与女性患者两个独立序列。
