根据患者真实数据,自动生成个性化健康处方和评估报告,为患者提供更具操作性的建议。
诊后支持通过算法识别检查项目异常及转诊条件,自动生成追溯与转诊任务,助医生及时发现患者异常,推动分级诊疗。
The AI Chronic Disease Management System - Multi-morbidity Management is a digital system designed to help primary care facilities accurately identify chronic disease patients in the region and provide standardized, continuous chronic disease management services.
It integrates medical and public health services through standardized pathways, assisting primary care facilities in providing homogeneous and continuous chronic disease management services for patients. It meets the construction requirements of integrated chronic disease clinics in primary care facilities, and realizes chronic disease management integrating medical care and disease prevention.
Key functions include:
1. Simultaneously connect public health and Hospital Information System (HIS) data to build a dynamically updated dedicated chronic disease patient database. Automatically screen high-risk or under-management patients of various diseases via algorithms, accurately locate the target population, and greatly reduce the patient screening cost of medical institutions.
2. Divide chronic disease management into multiple management nodes including pre-consultation, in-consultation and post-consultation, and standardize medical practice through checklist-based standardized pathways.
3. Pre-consultation intelligent triage: connect to intelligent measuring devices to collect vital signs data in real time. During consultation, prompt mandatory and optional laboratory and examination items in accordance with clinical guidelines, and output clinical decision assistance via built-in algorithms.
4. During consultation, users can quickly complete follow-up tasks for hypertension and diabetes, as well as conduct follow-up for other chronic diseases and formulate weight management plans. The system automatically extracts and fills in pre-consultation data, and supports combined follow-up mode.
5. Generate personalized health prescriptions and assessment reports automatically based on patients' real data, providing patients with more actionable recommendations.
6. Post-consultation support: identify abnormal examination items and referral criteria via algorithms, automatically generate follow-up and referral tasks, help doctors detect patient abnormalities in a timely manner, and promote hierarchical medical treatment.
