Primary epiploic appendagitis are rare causes of acute abdomen. It results from a torsion of an epiploic appendix, with spontaneous ischemia or venous thrombosis. We report the case of a 34-year-old patient who consults for intense pain in the right iliac fossa with nausea and constipation, but without fever. The physical examination shows a defense of the right iliac fossa. The CT scan shows a significant infiltration on the anti-mesenteric edge of the cecum. The diagnosis of primary epiploic appendagitis is made and the patient is progressing well under medical treatment.
