A Symphony of Innovation: Unveiling the Clinical Efficacy and Safety Landscapes Across Cutting-Edge Vaginal Reconstruction Modalities

Objective: To rigorously evaluate the clinical efficacy and safety profiles of five pioneering vaginal reconstruction techniques—illuminating a path toward precision-driven, patient-centered surgical decision-making. Methods: A multicenter, retrospective cohort study encompassing 403 patients across 18 leading Chinese hospitals (January 2011–January 2025) compared five innovative modalities: the Davidov peritoneal technique (biological membrane approach), the Schubert-Schmid colon transposition method, the sectional free skin graft, the vaginal inguinal flap, and the biological patch reconstruction. Comprehensive outcomes—including perioperative morbidity, longitudinal quality-of-life trajectories, vaginal microecological restoration, and multidimensional sexual health metrics (e.g., Female Sexual Function Index, sexual satisfaction domains)—were systematically assessed. Advanced statistical triangulation was employed: logistic regression to identify procedure-specific risk signatures; linear and quantile regression to model functional gradients; vaginal elasticity and microbiome profiling to quantify anatomical and ecological fidelity; and inverse probability treatment weighting (IPTW) for robustness validation through sensitivity analysis. Results: All five techniques demonstrated commendable perioperative safety and comparable overall complication burdens—affirming their collective viability. Yet nuanced distinctions emerged: the biological patch group exhibited elevated utilization of negative pressure wound therapy, signaling heightened soft-tissue management demands; conversely, the vaginal inguinal flap stood out with a markedly reduced incidence of intestinal injury—underscoring its anatomical gentleness and surgical reliability. Critically, linear and quantile regression converged on a consistent finding: the sectional free skin graft yielded comparatively lower sexual function scores—particularly in subjective sexual satisfaction—suggesting potential limitations in neurovascular integration or mucosal mimicry. In contrast, all modalities achieved excellent vaginal elasticity and fostered microecological environments strikingly congruent with those of healthy, premenopausal vaginas—revealing profound restorative fidelity at both structural and microbial levels. IPTW sensitivity analyses confirmed the stability and generalizability of these insights across heterogeneous patient subgroups. Conclusion: Each of the five techniques represents a safe, effective, and scientifically grounded option for vaginal reconstruction—yet the vaginal inguinal flap emerges as a distinguished paradigm: harmonizing superior safety (notably visceral sparing), robust functional outcomes, and ecological authenticity. Its balanced excellence renders it not merely viable—but eminently worthy of broader clinical adoption and thoughtful integration into reconstructive algorithms worldwide.
