Blinded appendix of raw data. This includes exemplar quotes extracted from qualitative patient interviews regarding the self-management document provided to them, as well as their responses to Likert-type or numerical questions. This appendix also includes the contents of the self-management guideline and interview questions that were asked to all participants.
