<b>Purpose:</b> Accurate identification of malignant thyroid nodules remains a clinical challenge, especially when biopsy results are inconclusive. Compared with benign thyroid nodules (BTN), papillary thyroid carcinoma (PTC) exhibits aberrant DNA methylation profiles. <b>Methods:</b> Genomic analyses were performed on tissue samples from early-stage PTC and BTN using the 850K methylation array and RNA sequencing. Identified genes were validated in two independent mass spectrometry-based case-control studies. <b>Results:</b> RUNX1 was hypomethylated in PTC, with all odds ratios (ORs) ≥ 1.50. RUNX1 methylation achieved favorable diagnostic accuracy in distinguishing early-stage PTC from BTN. <b>Conclusions:</b> Our study revealed the association between RUNX1 and PTC, indicating that RUNX1 methylation serves as a potential biomarker for companion diagnosis of malignant thyroid nodules.
