This series contains admission and discharge cards for patients and workers from Garrawarra Hospital, previously known as Waterfall Sanatorium.The admission and discharge cards contain the following information on front: name of patient, date of birth, occupation, religion, last address, birthplace, residence in NSW (number of years), residence in commonwealth (number of years), married (where, age, to whom) or single, father's name and occupation, mother's name and maiden name, if Briitsh subject (birth or naturalisation), hospital fund, nature of pension, where payable and no. of certificate, armed services (no, rank, unit), Children living (name, ages, addresses), number of children deceased (male or female), name and addresses of relatives or friends. Other side: admission number, admitted date, discharge date, reason, ward or dorm.Card information may slightly differ over time and admission cards arranged alphabetically by last name.
