All models contain controls for FS race and ethnicity. When the number of cases used to estimate each model changes between models it is because some independent variables predict the death certificate racial classification perfectly (e.g. there were no American Indians in certain occupational or income categories). Estimating all models using the smallest sample size does not substantively change the results (results available in online supplementary Materials S1).
