The BID Initiative implemented a multi-component immunization intervention, including digital tools and behavioral and organizational components, in collaboration with the governments of Tanzania and Zambia from 2013 to 2018. Data were collected using a structured survey of facility health care workers at baseline, midline (approximately 4 months post-implementation), and endline (approximately 12 months post-implementation). This dataset includes facility-level responses to questions about three data use scenarios related to DPT3 coverage, defaulters, and vaccine stock.
