Introduction: Chimeric antigen receptor T-cells (CAR-T) targeting CD19 represent a significant advance in treatment for patients with relapsed/refractory B-cell malignancies. Although a significant minority of recipients are women during their reproductive years, there is a paucity of data regarding pregnancy and neonatal outcomes in women previously treated with CAR-T. This is important as maternal T-cells are known to cross the placenta and into breastmilk during pregnancy and breastfeeding, respectively.
Case presentation: Here we present two successful pregnancies following CAR-T therapy where both neonates were initially breastfed. These represent the first cases of neonates born following CAR-T therapy comprehensively described in medical literature.
Conclusion: Pregnancy following CAR-T therapy does not appear to be associated with adverse neonatal outcomes. Further work is required to delineate the outcomes in this population.
引言:靶向CD19的嵌合抗原受体T细胞(Chimeric Antigen Receptor T-Cell, CAR-T)是复发/难治性B细胞恶性肿瘤患者治疗领域的重大进展。尽管有相当一部分接受治疗的患者为育龄女性,但目前针对既往接受过CAR-T治疗的女性群体的妊娠及新生儿结局相关研究数据仍较为匮乏。这一点具有重要临床意义,因为已知母体T细胞可分别在妊娠及哺乳期间穿过胎盘进入母乳。
病例报告:本文报告2例CAR-T治疗后成功妊娠的病例,两名新生儿均接受了早期母乳喂养。本案例为医学文献中首次全面报道的CAR-T治疗后分娩新生儿的病例。
结论:目前看来,CAR-T治疗后妊娠似乎并不会增加新生儿不良结局的发生风险。仍需开展进一步研究以明确该群体的妊娠与新生儿相关结局。
