失能人员调查问卷

本数据集统计了21家预防试验区（其中3家是应用示范区）有关日常生活能力量表的数据统计，合计1000余人，初步将21家合作机构中轻度、中度、重度失能人口划分出来，初步了解本项目所有合作机构中失能人口的类型及数量，并依据此做后续的量表评估及干预

This dataset compiles Activities of Daily Living (ADL) scale data from 21 prevention pilot zones, 3 of which serve as demonstration application zones, with a total sample size of over 1,000 individuals. It preliminarily classifies the mildly, moderately and severely functionally impaired populations among the 21 cooperating institutions, initially ascertains the types and quantities of functionally impaired individuals across all partner institutions of this project, and provides a foundational framework for subsequent scale-based assessments and targeted interventions.
