Selected Ambulatory care sensitive conditions (ACSCs) admission rate trend

ACSCs are those for which hospitalisation is thought to be avoidable with theapplication of public health interventions and early disease management,usually delivered in ambulatory setting such as primary care. High rates ofhospital admissions for ACSCs may provide indirect evidence of problems withpatient access to primary healthcare, inadequate skills and resources, ordisconnection with specialist services.

ACSCs（Ambulatory Care Sensitive Conditions，门诊医疗敏感病症）指可通过公共卫生干预与早期疾病管理避免住院的病症，此类干预与管理通常在门诊场景（如基层医疗）中开展。ACSCs相关住院率偏高，可间接反映患者在基层医疗服务可及性方面存在不足、医疗服务能力与资源配置欠缺，或是与专科医疗服务衔接不畅。
