This document presents an evaluation of the effect of a community intervention trial for the integrated active health management model for chronic diseases. Specifically, this study adopts a multicenter, prospective, open-label community intervention trial design to assess the efficacy of the integrated active health management model for chronic diseases in chronic disease prevention and control. The evaluation results indicate that this model can effectively improve the health status of community residents, reduce medical expenditure, enhance residents' health awareness and self-management capabilities, and strengthen the comprehensive service capacity of community health service institutions. Therefore, this model is worthy of promotion and application in the field of chronic disease prevention and control.
