De-identified outpatient prescription dataset of a national TCM master (Zhao Hanqing, 2025): 335 prescriptions with 5,951 herb-level records

Background: Complete outpatient herbal prescription records of Professor Zhao Hanqing, January–December 2025, TCM Internal Medicine department, Heniantang Clinic. Contents: 5,951 herb-level records; 335 prescriptions; 122 de-identified patients; 438 unique herb strings with doses; 91 disease labels; 102 syndrome-pattern labels. De-identified: sequential patient codes (P001–P122), prescription codes (RX001–RX335), ages grouped to bands, dates generalised to half-year periods (minimum k-anonymity = 3). Files: herb-level table (ZhaoHanqing_TCM_Prescriptions_2025.csv, 14 columns), prescription-level summary (ZhaoHanqing_Prescription_Summary_2025.csv, 12 columns), Data_Dictionary.csv, herb nomenclature QA notes, Frictionless Data Package metadata (datapackage.json). Reusability: UTF-8 CSV, no proprietary tools required. Suitable for co-prescription pattern mining, network analysis, and methodological studies in single-practitioner TCM settings.
