Current agreement on collaboration between LSS and adult rehabilitation, (Yes=1, No=0)

The municipality has a written agreement on how to cooperate in individual cases between LSS and adult rehabilitation. The agreement must also have been followed up over the past year. Management level refers to the level within the organisation that has the mandate to decide on agreements with adult rehabilitation. The agreement shall, on the one hand, describe a specific approach for the implementation of the collaboration and, on the other hand, specify the division of responsibilities. Collaboration in individual cases refers to overall joint action at an organisational level for a particular purpose. Someone or some adds their specific resources, competencies and/or knowledge to a task that they jointly have to accomplish for the individual. ‘Follow-up’ means that the actors concerned have systematically verified that the agreement in place for external cooperation is being used and functioning as intended and that it has been revised if necessary. The follow-up must be documented.
