Current procedure for information on coordinated individual plan (SIP) in financial assistance, (Yes=1, No=0) (-2021)

The municipality has written and management procedures decided that the individual should be informed of their rights to SIP and followed up these procedures. “Coordinated Individual Plan” (SIP) refers to a health and care plan that describes actions that the individual needs from both healthcare and social services and developed through coordinated care planning. Chapter 2, Section 7 of the SoL and Section 3f of the HSL. ‘Followed’ means that the municipality has checked that the routine is being used and works as intended and that it has been revised if necessary. The follow-up must be documented and dated.
