Care for dementia – multiprofessional dementia teams, (Yes=1, No=0)

Provides care within the framework of a multi-professional dementia team to people with dementia in at least one of the social services’ different forms of housing. Multi-professional team-based work with competencies from healthcare and social services aims to create continuity in care and care for people with dementia and to facilitate assessments that require multiple perspectives to cater for care and care from a holistic perspective.
