Research Background: Electroconvulsive therapy (ECT) is widely recognized as an effective and rapid treatment option for patients with major depressive disorder (MDD). However, the neural mechanisms underlying this therapy remain uncertain. Recent studies have indicated that the healthy brain operates near a critical state, which may reflect the balance between neuronal excitation and inhibition. Objective: In this study, we examined whether the critical state of MDD is altered, and if so, whether ECT can reverse these changes. Critical dynamics analysis was conducted on resting-state functional magnetic resonance imaging (rs-fMRI) data from 39 MDD patients and 38 healthy controls (HCs). Results: We found that compared with HCs, patients with MDD, particularly those with positive responses to ECT, tended to exhibit smaller mean avalanche sizes and lower branching ratios, indicating a subcritical state at both the whole-brain and functional network levels. Importantly, ECT effectively corrected this abnormality and enhanced the degree centrality and functional connectivity of high-degree nodes in networks including the default mode and frontoparietal networks. Conclusion: These findings demonstrate that ECT can modulate the large-scale brain dynamics of MDD patients to bring them closer to the critical state. Our study uncovers the pathological basis of MDD and the network-level mechanisms underlying the therapeutic effects of ECT.
