Unit survey, elderly care - Current routine is for scheduled meetings with nurses in matters concerning persons with need of municipal health care in special housing, elderly care, proportion (%)

For unit level, the value is given as Yes=1 and No=0, while at municipal level it is given as a percentage (%) of units where there are routines for how the planning of the individual's health care (care planning) is to be carried out in collaboration with the individual and with the municipal health care personnel and home care personnel. The procedure shall be decided at management level and documented. It must be up-to-date, i.e. followed up or drawn up within the last year.
