An implementation plan is a documented planning that describes how a decided effort should be implemented for the individual. The plan is up to date if it was designed less than 12 months ago or was followed up during the last 12 months, i.e. after 3 April T-1. The follow-up must have been recorded in the implementation plan or journal. Even if a person refuses to have a plan drawn up, a plan must be made of how the action is to be carried out.
