Case management models aim to coordinate care and support efforts and ensure that the individual has access to adequate care and support. Individual Case Management in the form of Strength’s model focuses on the individual’s abilities, opportunities and self-determination in terms of form, direction and content in planned and implemented interventions. The case manager supports the individual mainly through outreach work outside of constituency.
