Individual care plan for breast cancer, proportion (%) - Target level ≥ 100

Proportion of newly diagnosed breast cancer cases where an individual care plan (My Care Plan) has been drawn up in consultation with the patient. An individual written care plan, called My Care Plan, shall be drawn up for each patient with cancer according to the National Cancer Strategy (SOU 2009:11). The National Board of Health and Welfare has set 100% as the target level for the indicator.本指标为新确诊乳腺癌患者中，经与患者协商制定个性化照护方案（My Care Plan）的病例占比。根据《国家癌症战略》（SOU 2009:11），需为每位癌症患者制定名为My Care Plan的书面个性化照护方案。国家卫生与福利委员会已将该指标的目标值设定为100%。
