For unit level, the value is given as Yes=1 and No=0, while at municipal level it is given as a proportion (%) of units where routines are in place for scheduled meetings with nurses or other health care professionals, in matters concerning persons with need of municipal health care interventions. The procedure shall be decided at management level and documented. It must be up-to-date, i.e. followed up or drawn up within the last year.
